Assignment: Implementing a Public Health Program
Using the South University Online Library or the Internet, research about the ten leading causes of death in the United States. List all the diseases and select one disease. Research further on your selected disease.
Based on research, create a 5 to 7 pages Microsoft Word document that includes:
i. A proposal for a public health program to address that disease within your community.
ii. An outline of the steps you will undertake to evaluate the program you have proposed.
iii. An evaluation design for your program.
iv. An identification and collection of data to evaluate your program.
v. An explanation of the method determined for collecting the data.
vi. Conclusions from the data and justification of those conclusions.
vii. Recommendations for improvements to your program and a plan to communicate those recommendations.
Format your assignment according to the following formatting requirements:
i) The answer should be typed, using Times New Roman font (size 12), double spaced, with one-inch margins on all sides.
ii) The response also includes a cover page containing the title of the assignment, the student's name, the course title, and the date. The cover page is not included in the required page length.
iii) Also include a reference page. The Citations and references must follow APA format. The reference page is not included in the required page length.
