Functional Assessment Homework
For this homework you will be using the three assessment tools below on someone over the age of 65, or someone you believe has a "disability" of some kind, in order to assess their functional status. Katz Activities of Daily Living
a) Lawton Instrumental Activities of Daily Living Scale
b) Geriatric Depression Scale (Short Form)
Step A: Interview your "client" and gather information regarding their brief health history (age, gender, social history (marital status, occupation, smoking/drinking/drugs), current medications - name only, and brief past medical history; NO review of systems).
Step B: For each assessment tool:
a. Write a short paragraph summary of what the tool assesses
b. What were your "client's" results? (Do not include the actual forms).
Step C: Final paragraph: Write an overall summary of your "client's" functional status based on the results of the three survey tools.
Format your homework according to the following formatting requirements:
i) The answer should be typed, using Times New Roman font (size 12), double spaced, with one-inch margins on all sides.
ii) The response also includes a cover page containing the title of the homework, the student's name, the course title, and the date. The cover page is not included in the required page length.
iii) Also include a reference page. The Citations and references must follow APA format. The reference page is not included in the required page length.
