Pharmacotherapeutics of Gastrointestinal Disorders
For the diagnosis of gastroesophageal reflux disease (GERD), write a prescription for a medication used to treat GERD.
Be sure to indicate drug name, strength, amount per dose, route, frequency of dose, amount to dispense, allow generics, number of refills and any special instruction to the patient

c. APA format must be used for this paper. If you are unsure of your APA formatting ability, the KU Writing Center with tutorials and LIVE tutoring is available to you. Please see the "APA Templates and Tutorials" tab in the Classroom Menu on the left side of the classroom layout. There is a link to the KU Writing Center there. d. Note that the correct and required KU File Labeling Format MUST be followed. You can see this format in the "Tips for Success" which is posted in the Course Home announcements. It was also emailed to you prior to the start of class. 5 points will deducted under formatting in the Grading Rubric for assignments not labeled properly. I must download and open each of your assignments. If these are not labeled properly it is nearly impossible for me to keep them all straight.
e. Legal and proper prescription formatting MUST be followed. I have posted the following in Webliography which you can review in addition to your textbook:
i. Joint Commission "Do Not Use" list for Prescriptions
ii. World Health Organization-Guide to Writing Prescriptions
f. You MUST include:
i. all prescriber identifiers including your name, title, address, phone number, DEA and NPI numbers,
ii. at least two patient identifiers as per JCAHO requirements,
iii. date of the prescription,
iv. name of the medication, dosage strength, time of day to take the medication, total quantity to be dispensed, number of refills. PLEASE NOTE that "PRN refills is NOT acceptable. Please see above Joint Commission "Do Not Use" list for other No-No's when writing prescriptions.
v. If there is a warning which must be used, then write that on the prescription. Otherwise, provide the instructions and warnings you would give to your patient about the medication you are prescribing in a narrative paragraph form in your paper.
