Ms. Jeniffer, a 23 -year-old single Asian American woman, complains of depression with prominent hypersomnia, increased appetite, and low energy, worsening since she stopped taking bupropion about 1 month ago. She reports that during the prior year, she had three similar depressive episodes, as well as three-4 day episodes of increased irritability accompanied by excessive energy, over talkativeness, distractibility, decreased need for sleep (3 hours being sufficient rather than her usual 9 hours) physical agitation and impulsively. Hs. Lee reports a hx of Hashimoto thyroiditis, occasional use of "diet pills" and worsening of mood symptoms around her menstrual periods, but denies ever being psychotic or hospitalized for psychiatric reasons. She reports a history of treatment with sertraline, during which she developed suicidal ideation, and with bupropion, during which she developed increased irritability. Her mother has bipolar I disorder with psychotic mania, and her sister has bipolar II disorder.
