Living Environment
1. Youth continue to struggle with peer interactions, often engaging in bullying of peers. Sophia will insert herself into her peers' issues, usually attempting to mediate peer conflict in a negative way. Sophia will initiate or instigate conflict within the home, which causes arguments or physical confrontations. Sophia will attempt to tell staff what to do and claim that they are not aware of how to do their job. Sophia will argue with the staff in an attempt to seek direct attention from that staff member. Interpersonal Relationships
2. Sophia reports that she has several close friends at school that she enjoys spending time with and connecting with. At the current time, Sophia has not been able to identify an adult with whom she has a connection; she will continue to work to build a healthy positive connection with the Pre-adoptive mom. Support reports that her workers are supportive of her, and she can communicate and express herself with her CFT Team.
School Behaviors
3. Sophia has progresses significantly in school and has been able to maintain a high level within their behavior modification system. Within the last 90 days Sophia has had few instances of needing to be redirected, peer conflict and disrespect to staff and peers.
Academic
4. Sophia is doing well academically, she continues to progress education team reports that she is excelling in her classes. She recently underwent several evaluations, which include psychological testing, Occupational Therapy Testing, Physical Therapy Testing, as well as Academic achievement levels which included reading and math skills.
Learning Disability
5. In the area of English Language Art Sophia is currently working below grade level. Strengths are noted in the area of oral fluency, vocabulary, and reading comprehension. In Mathematics Sophia is currently working below grade level. Sophia has made improvements in the area of subtraction and adding more than two numbers at a time. In the area of science sophia is doing well, cooperating in class and noticed to be excelling. Same for Social Study. Sophia has improved significantly within the last month in physical education, noted to be participating and overall cooperative while in class.
Attachment
6. Youth has significant issues with attachment, youth is not able to identify anyone that she has a close relationship with outside of several friends at school. Youth tends to treat relationships as transactions in hopes to gain materialistic items, versus genuine connections. Youth does not seem to be putting any effort into attempting to connect with pre-adoptive mom, youth struggles with building healthy relationships.
7. Cultural Stress-
Youth seems to disconnect from her race, youth struggles to accept her race and will often times lie to others about her ethinicity. Youth identify as an African-American Female despite being bi-racial. Youth seems to identify this way when around peers, in an attempt to fit it within the other girls in the home.
