Jessica is a 24 year old Caucasian female who is self referred to you for substance use and addiction treatment. She reports that she found your agency on psychology today and was interested in receiving outpatient services. She reported she has Tricare insurance and comes from a military family. Upon assessing her, she discloses that she has a severe history of trauma to include sexual assault and physical abuse, she declines to provide any additional information on this area however she notes she has a fear of males and does not like to be "confronted or yelled at." She reports that she has been using marijuana daily up to 4x per day averaging about $10-15 worth each sitting. She has a history of opiates, cocaine and benzodiazepine use via snorting which required multiple cycles of residential treatment to enter sobriety. She also has a history of overdosing on fentayl 3 weeks prior to requesting services. She reports that she previously has been sober 3 years from these three substances however she has relapsed on marijuana 6 months ago and then recently relapsed on benzodiazepines and opiates for a period of 30 days after discovering that she was 2 months pregnant from an ex-boyfriend. She would like to avoid entering residential treatment and has 1 week of sobriety from the benzodiazepines and opiates (fentayl) however she is currently experiencing significant cravings, and she continues to smoke marijuana daily.
