Problem: The 58-year-old has had T2DM for 15 years. She is currently taking metformin, sitagliptin (Januvia), and pioglitazone (Actos). Her hemoglobin A1C has ranged from 8.5-11.1 within the past year. Her A1C today is 9.9. The APRN orders insulin glargine (Lantus) to be administered at bedtime for one week. She also advises the patient to check her blood sugars fasting before breakfast and to bring the record to the next visit in 7 days, when this regimen will be reevaluated.
