Problem

Mr. John Purata is an 84-year-old man who has returned to his home in a long-term care facility where you are looking after the ulcer on his sacrum, developed during his stay in acute care after hip replacement surgery. Mr. Purata has been too tired to get up for meals, and you have noticed that his nutritional status is declining. Although he is on a pressure-relief surface, Mr. Purata tends to lie on his back.

You have mentioned to him the importance of repositioning to assist in healing the wound, but he merely closes his eyes and tells you he is fine the way he is. When you and the other staff in the long-term care facility try to get him up to walk, Mr. Purata is hesitant and requires a great deal of coaching and encouragement to walk even a few steps. He then says he is too tired to go further and asks to go back to bed.

He is frequently lying in bed, with eyes closed and the lights dim. His daughter and son are concerned that he seems to have lost his "get up and go" attitude. The wound is not improving, and you are concerned.
