Mr. T. is a 45 y/o Middle Eastern male new to your practice. He has a PMH of HTN and tells you that he is followed by cardiology. He is unable to tell you why he goes to cardiology other than "for my blood pressure and my sugar". He states that he chews on pieces of sharp varthemia to "help with his diet". You ask him if he has diabetes and he becomes defensive and states angrily and aggressively that, "NO. Aren't you listening to me?" You ask him what medications he is taking and he is unable to tell you the names nor does he have the pill bottle with him. He "thinks" he gets it filled at CVS but sometimes he gets medications at Walgreens and RiteAid. You call the cardiologist who cannot speak with you and states through his MA that he will call you back later. You complete your exam of the patient and discuss your plan to speak with the cardiologist and to call the patient after. Two days later you are still trying to have a conversation with the cardiologist, have requested medical records from his office and you have still not received them. The patient is calling you today very upset and states "Don't you know what you are doing? How come I haven't heard from you? You are not taking care of me!"
