Client A has a diagnosis of schizophrenia, lives in supportive housing, and has been working with a case manager. During a home visit, the client told his case manager he drank a six-pack over the weekend. The client has never been treated for alcohol or substance abuse.
Client B is a 40-year-old female who presents with complaints of anxiety, insomnia, loss of appetite, and inability to focus for the past few weeks. She had a history of using PCP, alcohol, and marijuana in her 20s and marijuana and alcohol periodically during the last 10 years. However, she has not used anything after completing treatment 2 years ago. She has maintained employment as a teacher's aide.
Client C has been diagnosed as bipolar and paranoid schizophrenia and has responded to mood stabilizers in the past. The client is not homeless, moving from shelter to shelter, and has not been able to maintain employment due to cocaine and alcohol abuse. Client has been in four prior treatment programs but usually relapses within 60 days of discharge.
Client D was seen at the Abused Persons Program after being beaten up by her husband for the third time. She was prescribed Zoloft in the past but is on no medication. She admits to being unable to sleep, unable to focus, having lost her job, and using alcohol periodically to "calm" her nerves.
Client E is a 25-year-old man who was on medication as an adolescent for attention deficit disorder and depression. He has used alcohol and cocaine for the past 10 years. He was in substance abuse treatment three times during the past 5 years and maintains abstinence no longer than 3 months.
