Assignment Problem:

The aim of this Quality Improvement (QI) project focuses on reducing the incidence of pressure injuries by a minimum of 2 pressure related injuries each month over a 4-month period, by addressing the widespread differences associated with the "subjective" skin assessments conducted by inadequately trained nurses and utilizing the validated "Braden Scale" assessment tool.  In addition, this QI project also aims to effectively address the gap in nursing knowledge as it relates to the negative effects of "Microclimate" and its harsh impact on tissue deterioration and pressure injuries.  Over the years, the medical facility Wound Care Program has struggled to transition from Pressure Injury treatment to prevention.  Effective re-education, training, and standardizing the use of the Braden Scale to 95% of the staff, will reduce the "subjective" and often inaccurate visual assessments of skin color and tissue hydration by nurses, which increase the potential development of Pressure Injuries (PI) and poor patient care outcomes.
