Problem: Project Evaluation Plan
Step I: Review your strategic plan to implement the change proposal, the objectives, the outcomes, and listed resources.
Step II: Develop a process to evaluate the intervention if it were implemented.
Step III: Write a 150-250 word summary of the evaluation plan that will be used to evaluate your intervention.
Address the following in your summary:
i. What data was collected?
ii. What tool will be used to collect the data?
iii. Who will be responsible for collecting data?
iv. How will this data be communicated to the team?
The response must include a reference list. Using Times New Roman 12 pnt font, double-space, one-inch margins, and APA style of writing and citations.
