Problem:
Between December 2015 and March 2017, five incidents of employees falling on slippery surfaces in the parking garage of Tower ABC have been recorded. In 2 of the cases, employees missed time from work. One of the falls was deemed a critical injury and required surgery to repair a badly broken wrist. It appears that the incident occurrences are related to a water drainage pipe running along the ceiling of the garage that leaks water in front of the main door to the offices. Because the garage is exposed on all sides to the outside, the water easily freezes on the concrete floor when the weather is cold.
