What questions would you have for the off-going nurse

Problem

Learning objectives:

• Prioritize and implement nursing care for multiple patients.
• Effectively delegate appropriate cares to a nursing assistant.

Scenario:

You will be coming in on the day shift starting patient care at 0730.

You are caring for Susan Burns, 58 years old, who has chronic liver failure related to hepatitis and bipolar disorder. Susan was admitted yesterday with increasing ascites, nausea, and fatigue.

Your other patient is Ryan Hartmen, 42 years old, who has Crohn disease, celiac disease, and depression. He receives Remicade every 8 weeks and last received 6 weeks ago. He was admitted yesterday with dehydration and fatigue secondary to vomiting and diarrhea. Ryan continues to complain of nausea but is no longer vomiting.

PREP: You will be the only nurse caring for these patients. You will have a peer partnered with you who will be your nursing assistant.

A. Time management sheet for 0730-0830.

B. Think about what you will delegate to the nursing assistant and list out what you want the nursing assistant to do.

C. Be familiar with the medications you will be giving.

D. Based on the information you have who will you see first? This may change once you get report but it's good to start with a plan and be able to change if needed.

E. You will be receiving report at the beginning of your "shift." What questions would you have for the off-going nurse?

F. For each patient, pick the priority lab you want the result for first. It may not be listed on the orders. Form an SBAR for each patient during simulation. (You can at least have the background ready to go.)
