• Behavior change is very important in chronic care management. Most population health issues are chronic issues. Think about the transtheoretical model of behavior change in Chapter 8 of Population Health: Creating a Culture of Wellness.
o Do you think this model could easily be applied to your project? If not, is your project a process or systems change? What model could you utilize to drive change?
• Quality improvement not only includes the acute care and primary care settings, but also disease prevention and population initiatives under the domain of public health as the local, national, and international levels. Despite being so widely accepted, we still have strides to make. Understanding the factors that enable or influence adoption of quality improvement, as well as the barriers, is very important.
o Remember that as a leader without formal managerial authority, you can influence change as an opinion leader, champion, or boundary spanner. Consider how you may apply each to your role in your setting.
