Problem
For population health project paper, I'm writing about post discharge interventions with NP following up decreasing 30 day hospital readmissions. My chosen population is Heart failure patients aged 65 and over who live in VA.
One of the given questions from the paper is that how I would ask the community what it wants and needs to improve its quality of life and how I will identify key stakeholders.
Can anybody help me to find the answer for the question? I don't know how to begin with. Let me know if additional information needed.
