You will find the forms by going to "Business Services", then click on Business/UCC Filings then " Forms", scroll down to and click on "Uniform Commercial Code". After reviewing the forms answer the following five (5) questions: 1) What is the information sought in lines 1a, 3b and 4 of a UCC-1? 2) What is meant by "Individual Debtor?" 3) What form do you use if there are two (2) or more secured parties? 4) What is a UCC-3 used for? 5) What form do you use if line 8 on a UCC-3 does not provide sufficient space?
