Fill out an incident form to report an incident with JJ, a 42-year-old male client who has a vision impairment. During a therapy session at the service he fell after tripping on a loose edge of the carpet in the therapy room while you were supporting him. As a result of the fall he developed a small bump on the left side of his head just above his temple. The temple area was not bruised. He also experienced loss of movement to his left hand.
Straight after the fall, JJ was examined and given first aid, the workplace supervisor was contacted, his emergency contact was contacted, and the ambulance was called to take him to the hospital for further investigation.
