In recent year, we have witnessed several cases of major collapse of engineering structures around the world not attributed to natural disasters such as hurricanes, tornados, tsunami, and earthquake but caused in part by engineers' irresponsible actions, corruption, bribery, etc. Use internet search to find one case of the major failure of engineering structures such as buildings, bridges, towers, plants, processes, etc. For example, in February 2017, nearly 200, 000 people living below the tallest dam in the United States were asked to evacuate as a spillway appeared to be close to collapse. Discuss the major causes and contributing factors of one major failure that occurred in year 2016-2017. What do you think should have been done differently from engineering and ethical perspectives to prevent the failure?
