You are working as a HIM Director and the Risk Manager has asked you to help address documentation issues that may be related to an increase in potential litigation. Physicians have been resistant to requests for improved documentation, rarely answered queries and they stated they didn't have time for "administrative tasks" and were busy taking care of patients.
