Problem

In June 1991, 11-year old Jaycee Dugard was kidnapped near her home in Lake Tahoe, California, by convicted rapist Phillip Garrido, and his wife, Linda. Jaycee was repeatedly raped by Phillip Garrido and held captive for 18 years, giving birth to two children during her ordeal. Jaycee finally escaped in August 2009. As tragic and horrible as these events were, it was even more devastating to learn that much of it could have, and should have, been prevented. Records show Phillip Garrido was under the supervision of the California Department of Corrections and Rehabilitation, which repeatedly made supervision errors that allowed Garrido's crime to go unnoticed and resulted in Jaycee's horrific ordeal.

The professor wanted to expand what this post should focus on his clarification is posted below:

I'd like for you to try and think past the emotion and individual officer failures and identify the implications of this case from the agency perspective. This might include topics such as training, supervisory review and oversight, quality control, caseload assignments, etc. If you were the head of the agency, what corrective actions would you take?
