Assignment: Psychotherapy
Select any of your patients at your clinical site to write the case formation. Ensure you include those 3 phases (Initial, Middle, and Termination). Do not forget to submit your paper to Safe-Assign prior to submission. Acceptable percentages should be lower than 25% match. Write a case conceptualization on any of your chosen pt. at the clinic.
i. Description of the Client's problem and provides a sophisticated depiction of all Pt's views; word choice conveys empathy with each perspective; illustrations succinctly contribute to coherent conceptualization. Please provide a detailed yet concise intro that identifies the client, age, ethnicity, symptoms, occupation, grade, etc. Descriptions set the context for the understanding problem.
ii. Identifying the client's vulnerabilities by considering why the client more likely to experience this problem(s) than another person.
iii. Identifying the client's triggers by considering the stimulus or source of the presenting problem(s).
iv. What is the pt. Core Beliefs?
v. What are the most central thoughts, feelings, memories, sensations, and situations that the client is avoiding or fused with? Differential diagnosis (Elaborate).
vi. Diagnosis (Elaborate)
vii. Exploring coping strategies by considering the ways in which the client deals with the effects of the presenting problem(s).
viii. Finally, describe a comprehensive treatment plan which considers the particular processes, interventions, and measures you might want to use with this particular client.
Format your assignment according to the following formatting requirements:
i) The answer should be typed, using Times New Roman font (size 12), double spaced, with one-inch margins on all sides.
