Update PHP / HTML form
I need the HTML modernized to HTML5.
The customer likewise has to fill in a phone number and email address, else the form should not validate.
As of now, it validates even if you don't type in a phone number and email address.
I would like to keep the captcha.
As well if you could delete unnecessary old code (some JavaScript).
