Three mechanisms:

1) Conduction - occurs in solids and stationary fluids (liquids or gases).

2) Convection - occurs in fluids. Change in the buoyancy of the fluid due to its change in temperature results in movement of the fluid.

3) Radiation - an electromagnetic wave form which therefore doesn't require any medium. Can occur across a vacuum.
