Problem: Send an email concerning, unsafe discharge process. Received a report that patient 445 is for discharge home, BLS pick up at 10 and that son is Donnell will be receiving the patient home. HHA set up is confirmed from 8-8pm but unfortunately no coverage from yesterday and the so apparently per note that BLS transport can be set up tom and he will be receiving the patient. Had conversation with the son today prior to discharge making sure there is somebody accepting the patient and he denies the conversation. He said he is not home and he still waiting for the HHA set up. Spoke with HHA agency and spoke with Janice Dean and she confirm that HHA is not set up and this is not gonna be a safe discharge.
