Search the QualityNet website, click on "Hospitals - Inpatient." On the left hand column find "Data Validation" and "Specification Manual." (You will have to scroll down to find "Specification Manual".) Click on the most recent version of the manual (for example 4.2b), then in section 2 Measurement Information and select a disease / condition to review. Also research section 10 CMS Outcome Measures which are based on claims submitted to Medicare.
