Problem: I need to review a service plan created by one of my case workers for Rocky, a 35- year-old male with Down syndrome He has lived with his 75-year old mother. Rocky is treated with Dilantin since childhood for mild frontal lobe seizure. His seizures have been controlled for several years. He has severe Hearing impairment and uses hearing aids which he dislikes and will time them out. He supplements his communication by using Basic American sign language. How do I improve on his need to develop independence, concern over his long-term placement and needs, Does not self-regulate medication and nee social outlets?
