One key point that really stuck out to me after reading "Human Error" by Dan Peterson is the basic causes of human error. Peterson narrowed the primary causes of human error down to three causes; work overload, a decision to err, and traps. Work overload is basically described as the employee taking on too much for him/her to handle, exceeding their limit. A decision to err is the employee choosing to perform an unsafe act. Reasons for this can include poor motivation, mental state, or the belief that an incident will not occur. The final cause is traps. Examples of traps include employee incompatibility, workplace design, and a poor safety culture within the company.
