Re-aim model and three dimensional cube model


Oral health and systemic health are dependent upon one another. A pilot project was developed to provide oral health promotion to children between the ages of 6 to 15 months of age. The target population was children in this age range who had at least one tooth or the eruption of the tooth was eminent, participated in Medicaid, had no other health conditions, and were enrolled patients in a comprehensive pediatric primary care clinic. Caregivers were asked about child dietary practices and home care practices. At each subsequent primary care visit the children’s oral health was assessed using microbial screening, child’s diet, home health practices and visualization of caries. Each child was allowed to participate for 24 months.
