Developing an Evidence-based Case Study

Aim:

The aim of this case study is to explore client education strategies for a client with a chronic illness. You will have the opportunity to utilise a case study to explore the aetiology, pathophysiology, risk factors and education of a client with a specified chronic illness. Your case study will include published research findings to inform your education of this client.

Task description :

This is an individual assessment item. You are required to choose one client scenario from the two options given below. You will then examine several key areas - for example the aetiology, pathophysiology, risk factors and management - in relation to that chronic illness and the client education with supporting evidence. You will demonstrate your ability to gather evidence from a number of recent and appropriate sources about a specified chronic illness. This research on the aetiology, pathophysiology, risk factors and client education will be specifically related to the client you have chosen.

Instructions:

Practical application: In this task you are required to do the following:

1) Provide a brief overview of the chosen chronic illness that includes: the aetiology, pathophysiology and risk factors of this illness.

2) Identify two specific topics of client education given by a Registered Nurse that would be required to assist the client to manage their illness.

3) Discuss no less than 2 specific, effective client education strategies that the Registered Nurse would use to assist the client to manage their particular illness and detail how this education would occur (i.e. at home, in a clinic, in a group and what tools would be used). These strategies must be supported by the literature

4) Your case study should demonstrate client and family-centred care that optimises individual self-management and promotes active participation of the individual and family in illness management.

Client Scenario

Peripheral Vascular Disease Case Study

Basseer Guzys, aged 64, presented at the emergency room after he developed severe pain, along with pallor and numbness in his right leg, below the knee. He underwent a Doppler study of his right lower extremity and was diagnosed as having an acute arterial embolism in his right popliteal artery.

An emergency embolectomy was performed to relieve the obstruction and restore circulation to his right leg. He was transferred to the surgical unit where you are a Registered Nurse, following the procedure. He has a history of malignant hypertension and arteriosclerotic cardiovascular disease. He has smoked 1-2 packs per day on and off for approximately 40 years but has now successfully quit smoking. He also has a 20 year history of prescription narcotic drug use related to a back injury. He is due for discharge home tomorrow morning.

Past Medical History:
Compression fracture of L 4-5 in 1996

Past Surgical History:
Appendectomy 1999

Social History:
Basseer is married but is estranged from his wife. He has three children and 2 of them live interstate. Ruby (33 yrs), River (35yrs) and Daniel (36 yrs) are his children. Daniel, his eldest son, lives close by and sees his father most weekends.

Family History:
Basseer's father died of a myocardial infarction (MI) at 56 years of age. Basseer's mother suffered from hypertension and died following a massive cerebrovascular accident (CVA) at 63 years of age.

Review of systems:
Basseer complains of increasing shortness of breath on exertion, daily headaches and palpitations.
He denies having any abdominal or chest pain and has not had any recent weight loss.
On examination, his observations are BP 160/100, P 97/min, R 22/min, T 37.1C

Medications:
He has been commenced on:
Crestor 20mg PO daily
Coversyl 10 mg PO daily
Sotalol 40 mg PO twice daily
