Problem: A 57-year-old female patient was sent to the hospital outpatient laboratory department by her physician with a written order for a hemoglobin A1c (HbA1c) test. The patient has known type 2 diabetes mellitus and polyneuropathy. She also has diabetic chronic kidney disease, stage 2.. These diagnoses were documented on the order form as the reason for the blood test. At the time of the test she was also diagnosed with oral thrush and given a prescription for Nystatin.
