Problem related to patients with chronic heart failure

In my healthcare experience, one common challenge is reducing hospital readmissions among patients with chronic heart failure. A data-driven method may help improve patient outcomes in this scenario. Information on patient demographics, medication adherence, vital signs, diet, and follow-up appointments will allow nurses and providers to see trends in the data that help determine which patients will be at high risk of readmission. This information would be found in electronic health records (EHRs) and remote monitoring systems so that providers can act immediately when worrying patterns are identified. McGonigle and Mastrian (2022) agree that informatics needs to provide a system to convert raw data into meaningful information, allowing clinicians to make informed decisions supported by evidence, to eventually improve the quality of care.

From this data, knowledge can be formed to design targeted interventions, such as personalised education programs, medication reminders, or home-based monitoring systems, to reduce preventable readmissions. A nurse leader is significant in interpreting the data, using good clinical insights, and making decisions to make interventions feasible, ethical, and patient-focused. Uncovering patterns and transforming them into action, nurse leaders make the care team implement solutions that meet the needs of the patients. According to Sweeney (2017), informatics helps healthcare providers not rely only on their intuition but to make evidence-based decisions. Nangle et al.
