Problem related to customer expectations


Assignment task:

A software project that meets customer expectations and is delivered on-time and within its budget is a rarity. Much more typical is a software project that, at best, does not meet user expectations, and at worst, simply does not work or is delivered very, very late. The intent of this question is to have you research, and think through, the different root causes of software failures by analyzing a software project that did not get delivered as promised, or failed in some other significant way after the project was being used by the customer.

Pick a software project or system that had a serious problem, before or after it was installed, and describe what you think were the root causes of the problem. These could be issues in the initial system design, weak development practices, a lack of testing, poor implementation, system maintenance problems, simple human error, incorrect capacity planning, lack of executive support, or simply not considering how people would really use the system. It could also be a combination of these factors plus the ones we discussed in class.

It is expected that you will need to do some independent research and analysis, and to make some assumptions about the software system when you answer this question since you will not have access to detailed information on the problem. You can pick a software problem that you are personally familiar with (from work or school), one you researched or have heard about, or one that is referenced in the list below. You should not pick a software project that is related to the Affordable Care Act (ACA), or a system failure that is related to a cybersecurity hack or data breach (such as Equifax), or a system hardware failure.

As part of your answer, you must describe the extent to which you think managements issues were one of the underlying reasons for the problem, and assign a percentage to this factor. (You will not receive full credit unless you include this analysis in your answer.) Make sure to provide a link to the software failure you are analyzing.
