Situation: You are working on the Peds floor and are asked to see an 18-month-old that was brought into the ED by guardian due to increasing respiratory distress. The patient was admitted for 24-hour observation. Background: 18-month-old born at term. Joshua has had symptoms of a respiratory infection for the past 48 hours. He has 2 siblings at home who have also had the same symptoms. Yesterday, Joshua's guardian took him to the pediatrician for the respiratory symptoms. He was then sent to the ED due to worsening respiratory distress and was admitted to the acute care pediatric unit for 24-hour observation. Patient is irritable and fussy, but consolable by guardian. Guardian has stepped away from bedside to make a phone call. Assessment: Over the past 12-hour shift, Joshua has shown no signs of improvement in his respiratory distress. He continues with tachypnea, mild retractions, and upper airway congestion. He is currently on room air sating greater than 90%. Recommendation: Continue to monitor respiratory effort and treat as needed. Plan to discharge once respiratory distress has improved. Guardian will need education prior to being discharged.
