Hospital readmission rates are an extremely important topic in the realm of healthcare. As you stated, high readmission rates impact both hospitals and patients. It is important to not only recognize the implications of high readmission rates, but to also reflect on why these readmissions are occurring. An article that I read explored how "Risk factors associated with avoidable readmissions include those related to patient, social, clinical and system factors such as patient behaviors, community services, adequacy and appropriateness of assessment and treatment, as well as accessibility and coordination within the healthcare delivery system" (Bamforth et al., 2021).
