Problem regarding diagnosis of major depressive disorder


Problem:

Write a response to: Given the information, I think that Henry may provisionally be given the diagnosis of Major Depressive Disorder (MDD). Henry meets the criteria for this diagnosis and specifically shows symptoms of depressed mood most of the day, insomnia, fatigue, markedly diminished interest or pleasure in all or most activities, and an increase in appetite. These five symptoms have also been present for over two consecutive weeks, which is a requirement of diagnosis. These symptoms cause clinically significant distress in Henry's functioning, are not attributable to substance use or medication, are not better explained by a different diagnosis, and are not accompanied by a manic/hypomanic episode. Therefore, I believe that MDD is the best diagnosis for Henry at this time. I would specify Henry's diagnosis of MDD as moderate because his symptoms are resulting in significant impairments to his education, relationships, and overall functionality. However, Henry seems to be able to manage part of his symptoms, as is evident in his continuing to attend school and eating snacks at night to escape. This places him between the "mild" and "severe" specifiers, which I believe qualify him as moderate. To feel more confident in my diagnosis of Henry, I would want to know the extent of his emotions as they relate to his relationships with his family. Does he experience excessive guilt?
