Problem:

History of Present Illness: Ms. TP is an 82 YOWF who presents with her son for geriatric evaluation. She reports recently she has had an increased urge to go to the bathroom. When prompted, she states the urge to urinate seems to come out of nowhere and sometimes she cannot make it to the bathroom. She states this happened to her for a while after she had her 8th child but she hasn't had an issue with incontinence for years. Past Medical History: 1. COPD 2. Depression 3. Mild dementia 4. Insomnia 5. Hypertension 6. Hyperlipidemia 7. B12 deficiency 8. Osteoporosis 9. History of falls Social History: • Smokes ½ pack of cigarettes per day • Multiple sexual partners, no condom use. Allergies: Reports nausea with penicillins Current Medications: 1. Alendronate 70 mg once weekly 2. Albuterol 0.83 mg/mL nebulizer treatment every 6 hours as needed 3. Spiriva Handihaler18mcg one inhalation once daily 4. Flovent MDI 44mcg one inhalation twice daily 5. Trazodone 100 mg at bedtime 6. Vitamin B12 1000 mcg IM once monthly 7. Paroxetine 10 mg daily 8. Atorvastatin 40 mg daily 9. Rivastigmine 4.6 mg patch Q24H 10. Lisinopril 20 mg daily 11. Hydrochlorothiazide
