Assignment task:

Rebecca has been an RPN for four years. She works in the community visiting clients in their homes. The clients that she visits have a variety of different needs. The care that she provides include: medication administration and medication reviews, at home chemotherapy, diabetes education and management, intravenous therapy, pain and symptom management and at home ventilation care.  Rebecca absolutely loves her job. She finds it very rewarding to support clients in the community striving to meet their optimal level of health and developing therapeutic relationships with them.  At this community agency the average number of client visits is 8-10/day. Rebecca has always been a very successful RPN in meeting the top range of the expectation for visits and sometimes accomplished more on a weekly basis. Clients continually provided the agency with positive feedback and praise for the care and support that she provided.  Rebecca has been going through a very volatile divorce and it is has really been taking a lot out of her emotionally and financially. She cannot afford to take time off and must continue to work in the appointments with her lawyer and counsellor during her work days. This have been very challenging for her as she is paid by the number of visits she completes. She knows that she has not been providing her usual level of care to her patients and feels very guilty about this.  Rebecca reflects about her week. One day she submitted and documented for 8 visits but she only did 6 because she was at a lawyer's appointment. Another day she skipped a treatment step in changing Mr. Black's dressing because it adds 15 extra minutes to the dressing change and when visiting Mrs. Jackson she was to assess, observe and evaluate her administering her own insulin but instead decided to give it herself because it was quicker. Both these shortcuts allowed her to increase her client visits that day. After reflection, Rebecca feels terrible about the quality of a nurse she had been. She does not feel very proud of her nursing practice and promises herself to never practice this way again. Part B is the Scholarly Essay which is weighted 18%
