Problem about improving community health outcomes


Problem:

Hawaii is one of the states with a high cost of living and is radically diverse. Housing insecurity, lack of medical insurance, limited English proficiency, and the high cost of commodities affect the community (Wu et al., 2025). These factors affect the community's health in various ways and significantly influence illness and disease outcomes (White-Williams et al., 2020). Furthermore, reproductive health remains a pressing issue in Hawaii due to reduced provider availability and increased travel distance to get to providers (Lee et al., 2025). Given these factors, potential health risks, such as the inability to attend and afford care, can arise, especially among vulnerable populations. One strategy to address SDOH in Hawaii is increasing awareness of programs such as the Hawai'i Homeless Outreach and Medical Education (HOME) project. The HOME project is a mobile, student-run clinic that provides outreach and free medical consultations to residents of Hawaii (Kurata et al., 2020). This project helps the community, especially those without insurance and who cannot afford healthcare. The Accountable Health Communities Health-Related Social Needs Screening Tool (AHC HRSN) is an appropriate tool for assessing SDOH. The screening tool examines the association of multiple health-related social needs (Sidebottom et al., 2025).
