Research chronic diseases, (diabetes), as well as the prevalence of high-risk behaviors seen in today's older adult population.
Prepare outline that identifies a health issue affecting the aging population, as well as the health promotion program your team will design to address that issue. In addition, prepare a reference list with at least 3 sources of information.
Include the following elements in your outline:
o Overview of your chosen health issue, including prevalence and effect on clients.
