Prepare a form with backend database for our website
Project Description:
We want to integrate a form that will request users on our website to fill in that form and these data should be saved on the back end of our website. The form should allow upload of 1 file as well as having a calendar to select a date. This Form should be integrated into our current website developed in ASP.
