POSCO Co. in China uses charcoal and swamp ore to produce crude iron in closed furnaces. The resulting reaction can produce sulfur dioxide (SD2) (A). SD2 can also be produced by oxidation of charcoal (B). On january 13th (the first day after Chinese New Year shutdown), a worker was overcome with SD2 in the furnace area and died. A safety inspector later pieced together the following facts: in a cost cutting measure over the holidays, the company had shut down all ventilation in the plant (C), contrary to company policy, the worker was not wearing an appropriate respirator (E) and did not attach a SD2 warning monitor to his shirt (it was still in the case D).
