Fail safing (or pokayoke) carries with it an altered view of people and their performance:
a - humans are inherently variable and deserve to be protected from their own natural tendencies to vary
b - human mishaps are unnatural; publicizing this is the best way to eliminate them
c - the task of eliminating human mishaps is best divided up, with inspectors having the main job of detecting mishaps, and various experts designing solutions
d - with practice and repetition, human mishaps tend to be extinguished
