Jason is 11 years old and has Type 1 Diabetes. He was diagnosed with Diabetes when he was five years old. He is comfortable taking his "blood sugar" readings during the day. He participates in sports and is on the soccer team, which practices thrice weekly after school. His mother always packs his lunch for school so he does not have to worry about eating the food in the cafeteria.

Written meal plan with snacks -Breakfast (3pts) -Lunch (3 pts) -Dinner (3 pts) -After School Snacks on Soccer Days (these are different than snacks during the day and before bed (2 pts) -Snacks during the day and before bed (2 pts)
