Problem: Make this more formal: CM verified member was discharged on 5/18/26. Currently has active UTI and unable to take her chemo medications until active infection has resolved. Sandy informs that member is drinking a lot of cranberry juice. CM inquired if member is independently providing perineal care currently and Per Sandy member is very weak and she is trying to care for herself. CM advised Sandy to aid with continent care and to make sure the pads are changed regularly. CM educated Sady on proper perineal care to prevent UTI and encourage fluids unless contraindicated. Concern: Sandy informed that member's pain is progressing and member is currently on routine morphine and as needed pain management but pain in not manageable. She will be in contact with m ember's PCP within the VA to get refills for her medications. Additionally, in home palliative team, has not reached out for continuity of care and VA will be providing the Palliative support. Accent care has started PT and no nursing support currently to manage UTI. f/u PCP scheduled for today.
