Patient been diagnosed with moderate asthma


Problem:

Lorna Lungley is a 25-year-old exercise instructor who has been diagnosed with moderate asthma. She is using her prescribed inhaled ICS with formoterol as needed for exacerbations to relieve her cough and wheeze with reported satisfactory clinical effect. Currently, she needs to use her medicine about once monthly for symptom control and is requesting a prescription refill. The CRNP considers the current guidelines and as she make the following assessment and plan of care: • The patient's asthma is well controlled, It is OK to refill as requested • The patient has excessive medication use is at risk for asthma death. Do not refill this medication. Refer to pulmonology. The patient's asthma is not well controlled and leukotriene modifier should be added along with albuterol pr use. The patient needs referral for immunotherapy to diagnose potential allergy triggers
