List the steps or the components in change model or theory

Problem

Change Models: (Prochaska and DiClemente's model of change behavior, Kurt Lewin's classic theory of change).

Two examples of organizational theories of change by examining two behavioral theories that are frequently used to support the process of change. Prochaska and DiClemente's model of change behavior was developed originally for use with patients to change health behaviors. Specifically, research specific to smokers and therapy informed this theory. Kurt Lewin's classic theory of change is a three-phase model that views change as a dynamic balance of forces, the driving forces, and the restraining forces.

Select one of the organizational or behavioral change models (Prochaska and DiClemente's model of change behavior, Kurt Lewin's classic theory of change) which were used successfully for evidence-based practice change in healthcare for many years, and:

1) List the steps or the components in the change model or theory.

2) Does the model or theory contain a component for appraising the evidence?

3) Does the model or theory contain a component for networking with the stakeholders during all phases of practice change?

4) Does the model contain components for identifying barriers and addressing barriers to implementation?

5) How the theory applies to the lung cancer problem?

The response should include a reference list. Using double-space, Times New Roman 12 pnt font, one-inch margins, and APA style of writing and citations.
