Once you finish, your manager asked you to write-up your findings so she can share them with the hospital's leadership team.
Task
A. Research "informed consent" and list the required components of an informed consent form for a radiography procedure.
B. Perform an internet search and find good examples of informed consent forms used in the imaging departments of at least two different healthcare organizations. For each example, provide the following information:
C. List the organization's name and describe the type of facility.
D. Provide a link to the website where you found the form.
E. Describe (or provide a copy of) the form.
F. Identify the type of imaging procedure for which the form was intended.
