He has smoked two packs of cigarettes per day for many years and comes to the emergency department with a productive cough, shortness of breath, and a fever. His sputum is yellowish green and his oxygen levels are low. He has a poor appetite and has lost 15 pounds in the past 2 months. He is considered malnourished for his 6'2 height. He has a draining blister on his left foot that he developed after wearing some new boots. He lost his wife 3 weeks ago after 52 years of marriage and says he cries most of the time.
