**This information MUST be individualized for each client.

Operational Definition of Behavior: Describe topographies (forms) of behavior in a way that can be easily understood by a novel observer. Need Assignment Help?

Interpretation and Analysis of Function of Behavior: State the function of the behavior and how this was determined. What environmental events typically precede this behavior? In what settings and with which people is it more and less likely to occur? What consequences typically follow the behavior currently? (e.g., do parents allow him to escape tasks? Is he provided access to tangibles? Etc.)

Proactive Strategies: What are you doing to prevent this behavior? (Pairing procedures, behavioral momentum, demand fading, visual cues, etc.).

Replacement Behaviors: What functionally-equivalent replacement behaviors are you teaching the client? (e.g., functional communication training - manding for a break for escape-maintained behavior.)

***NOTE: ALL replacement behaviors listed need to be a Reactive/De-escalation Strategies: What strategies will be used when this behavior occurs? In the case of dangerous behaviors (aggression, SIB), how will safety of the client and others be ensured?
